A copy of your child’s medical record can be requested by you at any time.  All requests must be in writing.  We have standard forms based on patient's age  that you may complete for this purpose or you may compose a letter yourself.  There is no charge should you request only a copy of your child’s immunization records and a flat fee of $20 per child for complete medical record.  Please allow seven (7) business days for preparation of the records.  There is an additional flat fee of $5 for postage if you would like your copy mailed to you or another physician’s office.  Please note that we do not copy medical records you may have provided to us from another facility or physician including tests, procedures, consults, or office visit notes.  Please contact that facility directly for transfer of those medical records.
