Forms (camp/school/daycare)

We understand that school and camp forms are required for enrollment in many programs and we are happy to complete your child’s forms for you.  There is no fee for completion of the necessary school and daycare forms, but as camp participation is optional there is a $15 charge per form.  We strive to complete forms that are brought during your routine check up immediately.  Forms that are dropped off, faxed or sent by mail are often completed within 2 to 3 business days but we ask that you allow up to one week for completion in the event of high volume.  Your child must have had a physical exam within one calendar year for us to complete their forms.  We also require that the parents’ section be filled out properly before submission since we review your portion before completing our section.  Please provide a self-addressed envelope with appropriate postage if you would like the completed forms to be mailed or a phone number we may call when they are ready to be picked up during routine office hours.
